CC: Stubborn weight gain, chronic low energy
Sx: Cold hands and feet, loose stools, poor memory, poor motivation/ accountability regarding healthy diet and exercise
Hx: This woman has tried and failed many diets and exercise regimes. She has such low energy she finds it hard to commit to making any changes in her life.
Other Info:
- Currently she is not eating very much. Despite a low intake, she finds herself putting on weight. She feels like simply looking at food will cause her to gain weight.
- As she gains more weight, exercise is more uncomfortable and difficult.Â
- Her body temperature has been low for as long as she can remember.
- She feels like her thinking is clouded and is very forgetful.
- The most she can do every day is get up, get showered and dressed, get to work, and maybe make herself dinner. She often falls asleep watching TV shortly after dinner.
- She wants to feel in control of her life again. She is likely going to need a lot of support and claims that she will do better with small systematic changes.Â
